Principles of Neurosurgery fills a unique niche in the world of neurosurgery textbooks. Already the authors of an extensive multivolume textbook in the field, Rengachary and Wilkins have endeavored in this edited textbook to produce a “book for beginners.” They have succeeded admirably. Their book is an excellent introduction to most of the basic principles and problems of neurosurgery. To quote the editors, it is “comprehensive without being encyclopedic.” The book begins nicely with a chapter on the history of neurosurgery. Basic scientific principles are introduced, followed by chapters on intracranial pressure, the blood-brain barrier, cerebral edema, and brain herniation, as well as one on states of altered consciousness. Subsequent chapters cover the spectrum of typical problems that the neurosurgeon encounters, such as hydrocephalus, congenital anomalies, traumatic head and spine injury, vascular disease, neuro-oncology, spondylosis and disk disease, peripheral-nerve disorders, and neurosurgical approaches to pain. Newer neurosurgical techniques such as stereotactic radiosurgery and stereotactic surgery are included, and there are valuable chapters on endovascular techniques and radiation therapy.

Overall, the chapters are very well written, with attention to basic tenets, although there is occasionally a tendency to oversimplify complex problems. For a multiauthored textbook, it has surprisingly little redundancy -- a tribute to the editors.

There are many photographs, drawings, tables, and management algorithms. Color is used extensively throughout the book -- at times to the point of distraction. Given the pictorial emphasis of this textbook, I was surprised that pictures of patients with the various craniosynostoses were not included. The only truly disappointing chapter in the book is that on ischemic vascular disease. Its lack of details about the clinical features of stroke and stroke syndromes (amaurosis fugax is not mentioned once) leaves an important gap. Although the chapters on gunshot wounds to the head and maxillofacial fractures were both very interesting and informative, I would have preferred to see chapters on spinal epidural abscess (a true neurosurgical emergency), pseudotumor cerebri, and neurosurgical approaches to patients with AIDS. Principles of Neurosurgery is ideally suited to medical students and junior residents in neurosurgery. (Chief residents with whom I shared the book felt it was too basic for their level of training.)
